This game is simple. It is a game of luck. Below are 5 doors, each assigned a number. Choose one, fill out the form at the bottom, and see what you get. REMEMBER: You may only play ONCE per round. Do not keep trying different numbers.

- 1 door will give you nothing.
- 2 doors will give you a UL ticket.
- 2 doors will give you one random card worth 1.
